That example by jwiewel was cute. And it actually happened once at the old Lunt Avenue club in Chicago. On this occasion I was not the TD.

There were (for example) 20 players. Thus 1 would play 11, 2 would play 12, etc in the first round. The GM inadvertently paired as unrated would be number 20, and would be paired against number 10.

With the rating corrected, the GM would be number 1, so he would be paired against number 11. But 11 was the former 10, and 1 was the former 20, so the pairing was the same! And the pairings were the same for all the other players, as well.

In those days we used pairing cards and handwritten wall charts. So the handwritten wall chart had to be completely redone while round 1 was being played. No problem, though.
