Group membership rates

Is there a place on the US Chess website that lists what the current Group Membership rates are and what the age cutoff is? All I could find is this, https://www.uschess.org/index.php/Resources/Group-Membership-Program.html, but the link to the Group Membership rates doesn’t work, and the Group Membership information page doesn’t state what the age cutoff is.
